Code for America, the Open Knowledge Foundation, and other organizations have a bunch of open-source apps that are reusable and directly applicable to civic hacking.

Example tools:

Adopt-a-Hydrant – allow residents to adopt some kind of civic infrastructure, like hydrants.

Shareabouts – a mapping application for crowdsourced info gathering

CKAN – an application for hosting data
